(I don't know if this is an Everything issue?)
File was created in to %userprofile%/documents/
somehow, a file i was messing with...
[something] got put into %userprofile%/documents/ ???
%userprofile% is on my system (Windows) volume (E:)
i don't put anything there.
i don't "use" anything there.
i had edited (avidemux) a file, placing cuts into c:/out/
i renamed 1 of those cuts (using Everything)
i touched the file date (using Everything)
after that, after i had cut, after & had renamed & touched
Index Journal (1.min later, or at least at the next minute mark)
shows a File Create, of the original source file, in %userprofile%/Documents/
it is there. it is an actual file (not a link or a ghost or anything like that).
hashes check.
i have no idea how or why?
Everything's "instalDir" is on C: not E:
(a long time back, there was issue with file, or was it ADS, being dropped into instalDir)
my renames & touch dealt with the cut file, not the original
- except - for selecting, Ctrl+C, my original as the source for my touch... <- maybe something there?
source file is NOT a LFN (185 chars total, path+name)
/during/ my rename (single file), there certainly were "too long" message(s),
but i paired it down before finally exiting the rename dialog
(ending c:/out/ name was 220 chars total, so at that point, certainly short enough)
during the rename, i added texts toward the beginning of the name - not caring about
overall length at that point, knowing that i was going to prune content from the end
parts of the filename.
might there be a sized rename buffer, & that buffer overflowed ? before i finally
had the name paired down & something with that cause this oddity, dropping a copy of
the (original) file into %userprofile% ?
(i tried arbitrarily renaming, again, along those lines, but couldn't duplicate...)
might it not have been Everything that did this? might it have been avidemux?

file date (modified) is same as source
(created & accessed were set to current)
might Set File Time (#get-date-modified:<#clipboard:>) have, have... ?
attempted to touch something it couldn't, & that caused the drop in %userprofile% ?
(i do all of the above, regularly. the only odd thing is that FileCreate in %userprofile%)

MAybe avidemux creates a backup in shell:personal ?

Setting the file time in Everything should not cause this issue.
Did you have the original file copied to the clipboard? (eg: Did you copy the file to the clipboard to set the file time in Everything?)
It sounds like the file was pasted somewhere, but ended up being copied to %userprofile%/documents/
